Where to File an Incident Report

Reporting Incidents & Injuries on Campus

Certain incidents occurring on university-owned property or involving university activities may result in an insurance claim and must be reported promptly.

Reportable Incidents to Risk Management

Please report the following incidents to Risk Management for review:
• Damage to university-owned property
• Auto accidents involving University-owned vehicles
• Injuries to faculty, staff, students, or non-employees occurring on university-owned property

Incidents can be reported using the Risk Management reporting form.

Employee Workplace Injuries

If a university employee is injured while performing job-related duties:
• Contact 24/7 Nurse Triage at 1-844-454-1143
• Notify the Office of Human Resources and submit a Workers’ Compensation claim

Sexual Misconduct, Harassment, or Discrimination
Individuals who experience or witness an incident involving sexual misconduct, harassment, or discrimination should contact the Office of Equal Opportunity and Title IX.
