The Office of Student Financial Aid now offers online forms that are entirely electronic! These forms allow you to complete them online, sign electronically and submit them without the need to separately print, mail, or fax them. The web browser Edge does not support this software.

Student and Parent forms:
Student and parent forms are forms that require the signature of both the student and a parent before the form can be electronically submitted.
- Getting started – the student clicks the yellow button on the top of the form.
- Student enters their university username and password.
- Student completes their portion of the form.
- Student clicks the yellow “Submit to Parent” button on the bottom of the form.
- Parent receives an email with a link to the form.
- Parent completes their portion of the form.
- Parent clicks the yellow “Parent Signature” button on the bottom of the form last page to sign electronically.
- Parent will be taken to AssureSign website to electronically sign the form with a computer mouse or stylus. Clicks Finish.
- Student receives an email with a link to sign the form; check your University Email for this (possibly in your junk folder).
- Student signs with a computer mouse or stylus electronically and clicks “Finish”
- Once signed it will be submitted electronically to our office!
- Student and Parent will receive a final email with the option to download the completed document.
Student only forms:
Student only forms are forms that only require the signature of the student before the form can be electronically submitted. There are two (2) variations of Student only forms depending on Federal Guidelines:
Electronic Signature Forms
- Getting started – click the yellow button on the top of the form.
- Enter your university username and password.
- Complete the form.
- Click the yellow “Student Signature” button on the bottom of the form.
- Student will be taken to AssureSign website to electronically sign the form with a computer mouse or stylus. Clicks Finish.
- Once signed it will be submitted electronically to our office!
- Student will receive a final email with the option to download the completed document; check your University Email for this.
Digital Signature Forms
- Getting started – click the yellow button on the top of the form.
- Enter your university username and password.
- Complete the form.
- Type your FULL NAME on the signature line
- Click the blue “Submit” button on the bottom of the form to send it to our office electronically.
- Student will receive a final email with the option to download the completed document; check your University Email for this.
Forms will be checked for accuracy before you can sign it. Allow 2-3 business days for your MyISU Portal to reflect the submitted document. If any additional information is required, an email will be sent to the students University email address.
