The Federal Direct Subsidized and/or Federal Direct Unsubsidized Loans will be accepted by the student through the MyISU Portal (excludes summer semester). After accepting your Federal Direct Subsidized and/or Federal Direct Unsubsidized Loans through your MyISU Portal, any adjustments to these loans or to request an Additional Unsubsidized Loan (due to PLUS denial) will require that you complete a request form below.
Note: Documents may be faxed, mailed, or brought in during office hours. Contact a financial aid counselor for help completing any form. The student's name and University ID number should be clearly written on every document submitted, including student and parent tax returns.
